What is a hepatologist and an infectious diseases specialist?

A hepatologist is a doctor who specialises in diseases of the liver, bile ducts and pancreas. They diagnose and treat conditions such as viral hepatitis, fatty liver disease, cirrhosis and liver cancer. They oversee the selection of patients for transplants and monitor patients following biological therapy.

An infectious diseases specialist is a doctor who diagnoses and treats diseases caused by viruses, bacteria, fungi and parasites. They manage patients with viral infections (hepatitis B and C, HIV), tropical infections and those who have been exposed to infectious agents. They advise on prevention and vaccinations.

These two fields often overlap – particularly in the case of viral liver diseases, where close collaboration between the two disciplines is essential.

Issues worth reporting

  • elevated liver function tests (ALT, AST, GGTP, bilirubin),
  • suspected or confirmed viral hepatitis (hepatitis B, C),
  • fatty liver disease (NAFLD, NASH) or cirrhosis,
  • pain or discomfort in the right upper quadrant,
  • chronic fatigue of unknown cause,
  • jaundice (yellowing of the skin or whites of the eyes),
  • enlargement of the liver or spleen,
  • abnormal results from imaging tests (ultrasound, elastography),
  • prolonged low-grade fever or fever of unknown origin,
  • suspected infectious diseases (e.g. mononucleosis, Lyme disease, parasitic infections),
  • contact with an infected person or exposure to infectious material,
  • preparation for travel (travel medicine, vaccinations),
  • diagnosis following return from countries with an increased risk of infection.

What happens during a visit to a hepatologist and a consultation with an infectious diseases specialist?

During the consultation, the doctor conducts a detailed medical history, which includes, amongst other things, symptoms, lifestyle, diet, current medication and previous illnesses.

The available test results – laboratory and imaging – are then analysed. If necessary, the doctor may order further diagnostic tests, such as:

  • blood tests,
  • serological tests,
  • virological tests,
  • abdominal ultrasound,
  • liver elastography,
  • specialist tests for infectious diseases.

Based on the information gathered, the doctor makes a diagnosis or suggests further tests, initiates treatment, provides advice on lifestyle and diet, and plans follow-up appointments and further management. Every consultation takes place in a calm atmosphere, taking the patient’s individual needs into account.

Not always. Elevated ALT or AST levels may be caused by factors such as diet, medication, alcohol, strenuous exercise or infection. However, it is advisable to discuss the results with a hepatologist to rule out any more serious causes.

Yes, in many cases, fatty liver disease can be reversed through lifestyle changes, diet and treatment. However, proper diagnosis and regular monitoring are essential.
